If you wish to use a traditional payment method, such as a personal or business check, please complete the application form below. Once your application has been reviewed and approved, you will be given the option to select Payment by Check during the payment process.
After selecting this option, you will have 14 days to mail your payment. Detailed mailing instructions, including the payment address, will be provided upon approval.
Important: Payment by check is not considered received until the check has been delivered and successfully processed.
